This is the exhaust elbow or water injection elbow connector screw. Exhaust elbows are a common cause of overheating and poor exhaust flow of water into the exhaust.
The exhaust and connectors on a D4 should be checked at least every 5 years by removing it and checking for carbon build-up and corrosion.
